The storyline is set in 1834 London, where a secret occult society known as the Brotherhood of Hecate unleashes a horde of horrifying mutants and demonic abominations upon the city. Players can choose to play as one of two distinct protagonists: Ignatius Blackward, an aging American priest and expert in the occult, or Nadia Blackstone, a nimble European acrobatic fighter trained in martial arts.
The gameplay focuses on aggressive third-person melee combat. Armed with weapons like rapiers, staves, and magical talismans, players hack and slash their way through hordes of werewolves, zombies, gargoyles, and twisted abominations across foggy cobblestone streets, sewers, and cemeteries. A core mechanic of the game is the Adrenaline Meter: players must continuously attack enemies to keep their adrenaline from dropping, as failure to do so causes their health to rapidly drain away, forcing a fast-paced, high-pressure playstyle.
